Here at Gail Zimmerman, MD we are always accepting new patients. Regardless of whether you are sick or are simply due for a regular check-up, we are happy to welcome you to our office. We know going to the doctor can be a little overwhelming at times, so we are happy to provide you with our new patient forms ahead of time so that you can fill them out prior to your appointment and save you time during your visit. We encourage you to fill them out on your portal or with Healow Check-In.
 

Remember to bring a list of any medications you may currently be taking as well as any previous medical records you may want to provide. Cancellation Policy

Patient Forms

In order to provide you with the highest level of care, our forms are very detailed and comprehensive. There are two ways to fill out these forms: on the online portal – you will get a link with your appointment confirmation email immediately after scheduling your appointment.

The second way is through Healow check in. You will get a reminder text and email two days prior to your appointment. You must click on to the enclosed link to confirm that you are attending your appointment and you will also be able to fill out forms.

Insurance & Payment

I believe that good medicine can not take place in ten minute appointments. As a result of this philosophy it is not possible for me to accept the limited reimbursement that most insurance companies offer for primary care. Therefore, I am not a participating provider with any insurance except traditional Medicare. However, our office will submit your insurance claims to assist you with out-of-network reimbursement. Those who have out-of-network benefits will receive reimbursement from their insurance company according to their schedules. If you have one of the new Medicare Advantage plans, you will need to discuss procedures with our staff.

Payment is expected at time of service. We accept all major credit cards and checks. Please note that our credit card provider adds on a 3% fee for credit card use.

Cancellation Policy

Since it is a policy of our practice to allow a longer block of time per appointment, we have fewer appointment slots, then many practices. Late cancellations and missed appointments can prevent other patients from receiving much-needed care. So please read the following policy carefully so that if you do need to cancel, we are able to fit in other patients that need care.

STANDARD APPOINTMENTS:

Standard appointments must be canceled by 10 AM on the BUSINESS day prior to your appointment. Please note we are closed on Wednesdays. 

 

If you do not cancel or reschedule a follow up or a regular office visit before 1 business day, we will charge $50 to your credit card on file.

 

  • No shows are an automatic late cancellation and will be subject to the fees listed above. 

  • Telehealth visits are subject to the same fees. 

  • Please be advised that if you are more than 15 mins late, it will be an automatic reschedule and may be subject to fees.

NEW PATIENT AND ANNUAL COMPREHENSIVE PHYSICAL EXAMS:

We only have a very limited number of slots for comprehensive annual and new patient exams per week. Annual physical appointments require a larger block of time and lab work in advance therefore those appointments must be canceled THREE BUSINESS days in advance so that we might schedule a new patient in that time slot.

If you do not cancel or reschedule annual physicals or new patient appointments before 3 business days, we will charge $150 to your credit card on file.

​

  • Monday appointments, cancel before 10 a.m. the Thursday prior.

  • Tuesday appointments, cancel before 10 a.m. the Friday prior.

  • Thursday appointments, cancel before 10 a.m. on Monday.

  • Friday appointment, cancel before 10 a.m. on Tuesday.

​

Additionally, if paperwork and/or lab work is required for your appointment, failure to have that done by your appointment time may also result in a no-show/cancellation fee.

​

The no-show/late cancellation fee will be billed to you directly, and it’s not covered by insurance. We understand that emergencies, arise, and in these cases you can contact us and speak directly to the office manager. Thank you for abiding by our policies and thereby helping us to provide the best, most timely care possible.
